Working through Muñoz & Company I helped lead the design, implementation, and strategic planning for SOMOS Community Care (formerly Advocate Communtiy Partners) to provide branding and marketing services, and to aid in the procurement of state funding for SOMOS Community Care. Our work included the initial branding/identity design, state funding procurement, mobile application development, social media/marketing campaign, and website development for the organization.
SOMOS Community Care is a new network of diverse, community-based primary care physicians, specialists and service providers that have come together to transform the delivery of healthcare in New York City. SOMOS is a network of more than 2,500 physicians and 800 providers who care for over one million Medicaid beneficiaries. SOMOS Community Care was the ONLY community-based, physician-led network of the 25 organizations funded by New York State in a five-year, $6+ billion effort to reduce avoidable hospitalizations by 25%.
